STAYING WELL AT HOME

For older people to remain well at home for as long as possible

We want older people and people living with dementia to receive the support they need to remain in their home for as long as possible, or return home quickly following a hospital admission.

To achieve this, we will work towards:

  • Improving the continuity of health and social care services delivered to older people
  • Improving choice and a sense of control when it comes to how, where and when they receive support from services
  • Improving access to relevant information so older people are aware of support available before needs escalate

We want older people and people living with dementia to participate in, contribute towards and access services within their communities.

  • Improving the accessibility of health and social care services within communities across our region
  • Developing inclusive communities, providing more opportunities for older people to participate in activities
  • Improving sharing of information and increasing the number of social opportunities available to older people

INFORMATION, ADVICE AND GUIDANCE

For older people to have access to the right information and guidance

We want older people and people living with dementia to have access to the information, advice and guidance they need.

To achieve this, we will work towards:

  • Improving awareness of the early stages of dementia, and how to access diagnosis and support
  • Improving access to information so people can better understand how they can support somebody with dementia
  • Developing safe spaces so people can attend services and activities in their communities
  • Creating a regional approach that focuses on person centered support
  • Ensuring there is equity of support available across our region

TAKING PART IN COMMUNITY LIFE

For older people to have opportunities to get involved in community activities

We want older people and people living with dementia to be supported to engage in their local community and have a quality of life where loneliness is minimised.

To achieve this, we will work towards:

  • Improving social connectedness and encourage intergenerational positive relationships
  • Ensuring older people and carers feel valued and listened to when accessing services, support and activities
  • Creating more accessible community activities for older people, and those with sensory loss, physical disabilities or mobility issues
  • Ensuring older people have access to information and advice about services and opportunities

LIVING INDEPENDENTLY

For older people to enjoy good health and wellbeing so they can remain independent

We want older people and people living with dementia to enjoy good health and wellbeing, and be supported to live independently for longer.

To achieve this, we will work towards:

  • Improving opportunities for home adaptions/ modifications that will allow people to stay in their own homes longer
  • Improving inclusion within communities to help people have a sense of purpose
  • Increasing opportunities to participate in physical activities tailored to older people’s needs
  • Raising awareness and understanding of issues associated with mental health and wellbeing in older age
  • Improving digital inclusion

Over 100 older people and professionals working with older people have helped to shape these priorities. We will review these priorities on an annual basis, and continue to involve older people, their families and professionals in implementing them.

Our Regional Adult’s Services Board will now review the actions needed to implement these priorities; working with professionals and people with lived experiences to improve services and support.
