INFORMATION, ADVICE AND GUIDANCE

For people to have access to the right information and guidance

We want people  to have access to the information, advice and guidance they need.

To achieve this, we will work towards:

  • Improving access to information
  • Developing safe spaces so people can attend services and activities in their communities
  • Creating a regional approach that focuses on person centered support
  • Ensuring there is equity of support available across our region
  • Ensure information is accessible and easy for people to understand, and people are better aware of the information available to them

COMMUNITY

Creating community based health and social care services for people

We want to improve the accessibility of community based health and social care services by improving access to specialist services close to home. We will also work towards developing inclusive communities, providing more opportunities for people to safely participate in activities.

To achieve this, we will work towards:

  • Improving social connectedness and encourage intergenerational positive relationships
  • Ensuring people feel valued and listened to when accessing services, support and activities
  • Creating more accessible and inclusive community activities for people, and those with sensory loss, physical disabilities or mobility issues
  • Ensuring people have access to information and advice about services and opportunities

Integrated health and social care services for people

We want to create integrated health and social care services that will support people through key transitional times in their life.

To do this, we need to ensure adequate time and thought is available to deliver services. We must also maximise opportunities for people to explore all possible options open to them.

ACCESSIBILITY IN DEMENTIA CARE

Creating accessible dementia services and support

We know people living with dementia and their carers would
like support and information tailored to meet their needs.

Accessibility is a key theme in the All Wales Dementia Care Pathway of Standards, that we are working towards meeting in Cwm Taf Morgannwg.

We want to adapt communication methods, services, and support so people can
access information, advice and care in a way that suits them.
