Stay Well @ Home 2 is located in Merthyr Tydfil and Rhondda Cynon Taf, and open between 8.30am and 8pm every day.
The service provides a direct link to assessment teams (known as Single Point of Access) by taking referrals from community health and social care professionals. This includes GPs, district nurses and the Welsh Ambulance Service.
If somebody is unwell but does not need to go into hospital, the person’s health or social care professional can ring the Single Point of Access team, and a tailored package of support can be agreed within four hours.
This means that the person will receive the right support and care to stay safe in their own home.

All interventions are subject to a two week review to ensure that outcomes for people are being achieved and any onward referrals are made.
